Applicants must demonstrate that a "substantially consistent high level of care" has been rendered in all affiliated facilities.

The council shall consider facility inspections, patient abuse investigations and other available information. Criteria: gravity of the violation, manner in which the applicant exercised supervisory responsibility and any remedial actions taken.
